Questionnaires let you collect structured information from participants — for example at intake or after an appointment. This article shows who can build questionnaires and how the builder works.

Prerequisites

  • The Questionnaires feature is enabled for your organization.
  • Only organization owners can create and edit templates — regardless of whether they hold the Practitioner or Medical Assistant role. Other medical assistants in your organization can view the Questionnaires area and work with responses, but cannot create or change templates. Professionals without owner status have no access there at all. More on this in Understand roles and permissions.

Create a questionnaire

  1. Open Questionnaires in the sidebar.
  2. Click New questionnaire.
  3. Give it a title and, optionally, an intro text.
  4. Link the questionnaire to an availability type — at most one questionnaire can be Active for the same availability type at a time.
  5. Add your questions using Add another question.

Question types

Type Description
Free text Open text field
Number Number field, optionally linked to a tracked health metric (e.g. weight)
Single choice Radio buttons, one answer
Multiple choice Checkboxes, multiple answers
Dropdown Select from a list
Attachment Participant uploads a file (PDF or image) as their answer

Any question can be marked as voluntary using the "Make this an optional question" option.

Follow-up questions (conditional logic)

For dropdown, single-choice, and multiple-choice questions, you can attach a follow-up question to a specific answer option — it only appears when a participant selects that option. Follow-up questions themselves can be of type free text, single choice, or multiple choice.

Status: Draft, Active, Inactive, Archived

  • Draft — editable, cannot be assigned yet.
  • Active — assigned to participants, but no longer editable.
  • Inactive — temporarily disabled and editable again.
  • Archived — permanently retired.

To change an active questionnaire, first set it to Inactive, or duplicate it using Clone and edit the copy.

Assign questionnaires

  • Automatically: If a questionnaire is linked to an availability type, it's automatically assigned to participants once they have an appointment of that type, or when they register.
  • Manually: From within a conversation with a participant, use Send questionnaire to send a specific active questionnaire.

Review and export responses

Open the questionnaire to review submitted responses. Completed responses can be exported as CSV or XLSX.

Troubleshooting

  • I can't activate a new questionnaire. Check whether another questionnaire is already active for the same availability type — deactivate it first.
  • I can't edit a questionnaire. Active questionnaires are read-only; set it to Inactive or duplicate it.
  • I don't see the "Questionnaires" area at all. Only organization owners and medical assistants have access; otherwise, ask your organization owner.
