If your organization wants to see documents from you before your appointment — for example a referral or prior test results — you can upload them directly from your appointment confirmation page.

Prerequisites

  • Your confirmation link from the appointment email
  • Access to the email inbox on file for the appointment

How to upload documents

  1. Open the link from your appointment email. It takes you to the appointment confirmation page with a Documents section.
  2. Click Send verification code there. You'll receive a 6-digit code by email.
  3. Enter the code on the page and click Unlock.
  4. Drag your files into the upload area or pick them from the file dialog, then submit the upload.

The code is valid for 10 minutes. If it expires or doesn't arrive, use Request a new code to get a new one.

Accepted file formats are JPEG, PNG, GIF and PDF, up to 25 MB each, with a maximum of 10 files per appointment. Your organization is notified by email once you've uploaded documents and can view them there. Files you've already uploaded are listed on the same page, where you can download or remove them.

After too many incorrect code entries, access is locked for security reasons and can no longer be unlocked with a new code. If this happens, contact your organization.

Document upload is available as long as your appointment hasn't been cancelled or completed. Depending on your organization, the page may also show a date until which uploaded documents remain available.

Common problems

  • Didn't receive a code? Check your spam folder and use Request a new code to get a new one.
  • Code expired? Just request a new one — the previous code stops working after 10 minutes.
  • Access locked? After too many failed attempts, requesting a new code is no longer possible. Contact your organization.
  • File rejected? Check the format (JPEG, PNG, GIF or PDF) and size (max 25 MB) of the file.
  • Can't find the upload area? It appears on the same page as your appointment confirmation — open the link from your appointment email again.
