The patient journal is an ongoing, internal notebook per patient — ideal for short progress notes between appointments.

Prerequisites

The patient journal must be enabled for your account.

Open the journal

  1. Open the overview for the patient in question.
  2. Select the File tab.

You can also reach the journal directly from an ongoing conversation or from the appointment detail page.

Create an entry

  1. Write your text in the Create entry… field.
  2. If needed, use Add attachment to attach a file, or Add text snippet to insert a prepared text template.
  3. Click Add (or use Ctrl/Cmd+Enter).

Journal entries are plain free-text notes with an optional file attachment — there are no separate entry types like "diagnosis" or "procedure". For structured diagnoses and measurements, use the medical profile instead.

Search and filter the history

Use the Search entries field to search for keywords. Use the time period filter to narrow the view, for example to "This month" or "Last year". Use Reset filters to clear all active filters.

Edit or delete an entry

Within a short time window after creating an entry, you can adjust it with Edit or remove it with Delete entry. Once that window has passed, the entry is final and can no longer be changed — add a new entry with the correction instead.

Copy an attachment from a message

If a message from your patient contains a file attachment, you can copy it directly into a new journal entry using Copy attachment to patient file, without having to upload the file again.

Who can see the entries?

The journal is visible only to doctors and medical assistants at your practice. Patients have no access to their own journal. Every time the journal is opened, it's logged internally.

If enabled for your account, journal entries are automatically translated into other languages, so multilingual teams can understand them too.

Common problems

  • I can no longer edit an entry. The editing window has already expired — add a new entry with the correction instead.
  • The "File" tab is missing. The patient journal may not be enabled for your account — contact your practice administrator or our support team.
  • I can't copy the attachment from a message. This action is only available for messages that include a file attachment.
