Alongside the master data, the patient record has two further sections: Documents for files such as findings or referrals, and Measurements for health values you track over time.
Prerequisites
The full patient record must be enabled for your account. These two sections are not part of the simple patient overview.
Upload a document
- Open the patient record for the patient in question and select the Documents section.
- Click Upload.
- Drag the file into the field or select it — as PDF, JPG, PNG, or WebP, up to 25 MB.
- Give it a Title, choose the Category (for example lab, imaging, report, referral), and optionally enter the Document date.
- Click Upload.
Documents are stored encrypted; every download is logged.
Use Download to retrieve a document, or the trash icon to remove it after confirming.
Record a measurement
- In the patient record, select the Measurements section.
- Click Record value.
- Choose the Measurement type (for example blood pressure, pulse, oxygen saturation, fasting blood glucose, weight, body temperature, or respiratory rate) and enter the Value. For blood pressure, also add the diastolic value.
- Enter when the value was Measured at, and choose the Source (for example consultation, patient-reported, device).
- Click Save.
Use Show list to see all recorded measurements in a filterable table, or Show charts for the history as a chart. Values outside the reference range are marked accordingly.
Deleting an entry
Both documents and measurements can only be deleted shortly after they're recorded. After that, they're final (audit trail) — record a new, corrected entry instead.
Who has access?
Doctors and medical assistants at your practice can view documents and measurements. To add or change them, medical assistants need extended permissions, depending on your account's configuration.
Common problems
- A document isn't accepted. Check the format (PDF, JPG, PNG, WebP) and size (max. 25 MB).
- I can no longer delete a measurement or document. The short deletion window has already passed — record a new entry instead.
- The Documents or Measurements sections are missing. Your account may only have the simple patient overview enabled — contact your practice administrator or our support team.
